If your Ceramic Pro product arrives and is unusable, please fill out the below form for review to have the item replaced/have credit issued as appropriate. It is best that you (the installer) complete the form below as you have access to the defective product, but please reach out to your rep if you need assistance with this form. All requests relating to defective product must be submitted via this form – email or phone call submissions will not be accepted. Once your form has been submitted, you will be issued a claim number – please retain and use this number in any communication relating to that claim. Please submit a separate form for each defective product.

Please retain all defective product, product will be issued a return label for return back to Ceramic Pro. We will require product to be in transit back to Ceramic Pro before any credit or replacement is issued. Please do not send product back unless it has been approved.
